You should bring copies of all pertinent medical records such as doctors’ notes and laboratory and imaging results, including actual films. This information helps your doctor to understand your medical history, make a diagnosis, and suggest treatment options.
You should also bring your insurance card, as well as a referral and precertification information, if applicable. If you have chosen to complete the electronic patient forms (request at team@parkavenuemedicine.com) in advance, please remember to bring them to your appointment.
When you schedule your visit, our appointment coordinators will tell you when you should arrive and how you can prepare. In general, please plan on arriving 30 minutes prior to your scheduled appointment time.

Many of these forms can be reviewed and completed prior to your appointment by visiting our patient forms.
Doctors provide coverage through out-of-network benefits in most major insurance plans, Medicare, Workers’ Compensation. Our appointment coordinators can review insurance information with you when scheduling your appointment.
If you are unable to keep your appointment, please email team@parkavenuemedicine.com 1-day before a clinic appointment or 7-days prior to a diagnostic test or procedure. If you miss your appointment and do not call to cancel, additional fees may apply.
You may receive services such as anesthesia, radiology testing, pathology, or other services. These doctors provide vital services and are involved in your care—even though you may not see them face-to-face—so there may be additional charges for their services.

For your convenience, our practices accept cash, personal checks, money orders, and credit cards including Visa, MasterCard, Discover, and American Express. We do not accept traveler’s checks. We will charge your credit card only for the amount you authorize. Payment plans also may be arranged through our onsite billing coordinator.
Yes, all of our practices use the patient portal. You can access it using a desktop or laptop computer, or download the Advanced MD app on your smart device.
By logging into the app with your account, you can:
schedule or change appointments, including virtual visits
view your test and lab results
access your medical records, medications, immunizations, and more
request prescription refills
send secure messages to your doctor’s office
access your billing statements and make payments
To receive a copy of your test results and medical records, please email a request at team@parkavenuemedicine.com. You will need to sign a consent form to release this information if your records are being sent to another office or hospital. You can also login to your Advanced MD app account to view recent test results.
Prescription refill requests can be submitted via team@parkavenuemedicine.com or call your doctor’s office to request a refill.
We have updated our visitor policies in response to the current outbreak of the 2019 novel coronavirus (COVID-19). The new policies apply to all Park Avenue Medicine & Surgery locations.
We understand the important role that family members and visitors play in a patient’s healing process, and aim to balance the needs of patients with maintaining a safe environment for all.
The following policies are currently in effect.
Visitors under the age of 15 are not permitted.
Visitors will be screened by nursing staff before being permitted to enter the clinic. Those who have symptoms will not be allowed into our patient care areas and will be asked to leave.
The number of visitors to each patient is limited to one visitor for each adult patient, two parents or guardians for each pediatric patient, and two visitors for each labor and delivery patient, limited to partners and grandparents.
In the Surgical Centers, each patient is allowed up to two visitors at the bedside, limited to parents or support persons.
At our ambulatory and outpatient locations, patients may be accompanied by one visitor. Exceptions may be made only for a patient aide or a family member. Any visitor who is coughing or shows signs of illness will be asked to leave.
Park Avenue Medicine & Surgery does not restrict, limit, or deny visitation privileges based on age, race, ethnicity, language, national origin, socioeconomic status, mental or physical ability, religion, sex, sexual orientation, or gender identity or expression.
Preferred visiting hours to ensure patient care and comfort are 9:00AM to 5:00PM.
Be aware that there may be times when the length of a visit is limited, or guests are restricted, based on the health and best interests of a patient. When this happens, it is at the discretion of the doctor. Patients are also allowed to restrict visitors.
Balloons are not permitted in the clinic for safety reasons. Flowers and live plants are not allowed on some units, so please ask the nursing staff before bringing such gifts. Laptops and other electronic devices are permitted, but please be aware that Park Avenue Medicine & Surgery is not responsible for loss or damage.
We ask guests to keep their voices low during visits so as not to bother other patients. You should also be considerate of your loved one’s personal preferences—he or she may like short visits, for example, or ask to have a day without guests. Patients know best how much social interaction they need while recovering.
If you plan to care for your loved one after he or she returns home, use your visits to observe our nurses and ask questions. This can help you feel more secure and confident after your loved one is released from the hospital.
Be respectful of the nurses and doctors, as well as your loved one, who may want privacy when being seen by the medical team.
Please be mindful that all Park Avenue Medicine & Surgery campuses, buildings, and exterior spaces are tobacco-free, including electronic cigarettes (e-cigarettes) and all other smoking devices. Smoking or the use of smoking devices is not permitted anywhere at Park Avenue Medicine & Surgery.
Most of all, consider the needs of the patient. People who are hospitalized need to rest in a relaxed environment. Whatever you can do to encourage your loved one’s recovery may allow him or her to return home quicker.
